Executive Overview
This report documents the outcomes,lessons and performance of the Foundation's Primary Healthcare Program during the 2024 reporting period.
The program aimed to improve access to essential healthcare services by strengthening frontline delivery systems and expanding the current workforce capacity at the local level.
Overall,the program met or exceeded key outcome targets thereby demonstrating measurable improvements in both service coverage and effectiveness of the healthcare workforce.
Program Objectives
The program was designed to achieve the following objectives;
1.Increase access to essential primary healthcare services
2.Improve the quality of care being delivered by the frontline health workers
3.Support locally-led health solutions
The foundation partnered with government health agencies and local NGOs to support integrated service delivery.
Key interventions included;
1.Training and redeployment of community health workers
2.Ensuring that drugs being taken by patients are approved and certified by national drug agencies
3.Supporting the use of digital health tools in patient tracking
This implementation emphasised more on local leadership and policies,system integration and alignment with national health strategies.
Key Outcomes
1st Outcome: Expanded Access To Healthcare Services
▪︎Over 6 million people had access to essential healthcare services through supported facilities
▪︎Facility-level service coverage reached more people in targeted regions
▪︎Rural and underserved communities showed the most gains
2nd Outcome: Improved Quality of Care
▪︎Trained health workers demonstrated improved diagnostic accuracy
•Patients follow-ups increased significantly
▪︎Adherence to treatments improved totally in multiple service areas
3rd Outcome: Strengthened Workforce Capacity
▪︎More than 45,000 frontline health workers recieved formal training
▪︎Retention rates increased mostly in high-rise districts
▪︎Supervision and mentoring systems were institutionalised
Monitoring and Measurement
Outcomes were measured using;
1.Routine health management information systems
2.Facility assessments and service delivery audits
3.Household surveys and patient follow-ups
4.Independent third-party evaluations
Both quantitative indicators and qualitative feedback were based on pre-informed program assessment.
Challenges and Adaptations
Key challenges included;
1.Supply chain disruptions
2.Workforce shortages in remote areas
3.Data gaps in early implementation stages
The program responded with contingency planning,provision of more digital tools and increased local partnerships to bring about the desired autonomy.
