Community Health Access Initiative
Horizon Equity Foundation
Evaluation Period: January - December 2024
Purpose of the Evaluation
This evaluation would assess the effectiveness,outcomes and lessons learnt from the Community Health Access Initiative —- a multi-year program designed to expand access of essential healthcare services in rural and underserved communities.
The evaluation will focus on understanding what worked,what didn't and how the program strategy can be refined to do even better in the future.
Evaluation Approach
The program made use of a mixed-method evaluation approach that combined quantitative performance data with qualitative insights from the program participants.
Key evaluation methods includes;
1.Analysis of data from routine service delivery
2.Facility-level performance assessments
3.Household surveys and patient follow-ups
4.Interviews with frontline health workers and local partners
5.Independent third-party evaluation reviews
This approach ensured the contextual understanding of the outcomes.
Summary of The Key Achievements
1.Improved Access to Services
The initiative brought about a measurable increase in access to primary healthcare services across all target regions.Supported facilities reported higher utilisation rates, particularly amongst women and children.
2.Quality of Care
Evaluation findings showed improvements in quality of services rendered,including better adherence to clinical guidelines and improved patient follow-ups practices.Health workers demonstrated increased confidence and competence following training interventions.
3.Workforce Capacity and Retention
Training and supervision led to improved workforce stability.Retention of health workers improved in areas of critical concern while supervision systems became more reliable and structured.
Mentorship models introduced during the program were also identified as one of the key contributors to these outcomes.
4.Equity and Inclusion Considerations
The evaluation placed specific emphasis on equity outcomes.Data indicated improved service delivery amongst marginalised populations,including rural communities and low-income households.
Local partnerships played a critical role in reaching populations historically excluded from formal health systems.
Challenges Identified
Several challenges affected the program implementation;
1.Supply chain disruptions affecting the transportation and supply of essential medical equipment and medicines
2.Workforce shortages in remote areas
3.Initial data quality gaps during early implementation
Adaptive management strategies helped mitigate these challenges over time.
Lessons Learnt
Key lessons from the evaluation includes;
1.Integrated system-level approaches produced more results
2.Local leadership improved the relevance and uptake of these programs
3.Data systems requires continuous investments
Implications For Future Programming
Based on evaluation findings, the Foundation will prioritise on;
1.Strengthening it's local partnerships
2.Scaling successful interventions to more additional marginalised regions.
